$20.98 - $26.23 HourlyWhat We ProvideReferral bonus opportunitiesGenerous paid time off (PTO) , starting at 2 0 days of paid time off and 9 company holidaysHealth insurance plan for you and your loved ones, Medical, Dental, Vision, Life and DisabilityEmployer-matched retirement saving fundsPersonal and financial wellness programsPre-tax flexible spending accounts (FSAs) for healthcare and dependent careGenerous tuition reimbursement for qualifying degreesOpportunities for professional growth and career advancementInternal mobility, CEU credits, and advancement opportunitiesInterdisciplinary network of colleagues through the VNS Health Social Services Community of ProfessionalsWhat You Will DoApplies mutually shared and lived experiences navigating the systems of care as a caregiverServes as a role model, advocate, and mentor for children and familiesAssists with obtaining and maintaining clients' entitlements, including Medicaid, Public Assistance, SSI as well as assisting with problems related to housing or utilitiesInterviews clients and significant others, when appropriate , in order to obtain personal histories and to understand the social and emotional factors related to the client's mental health problemsParticipates in frequent active engagement with families to engage them in receiving services and assists in empowering them to take the steps necessary to maximize their quality of lifeDevelops inventory of resources that will meet the clients' needs as identified in the assessment process.Advocates for and assists clients in utilizing adjunctive community resourcesArranges transportation and accompanies clients to appropriate facilities/agencies, as necessaryProvides linkage, coordination with, referral to and follow-up with other service providersMonitors appointments medical facilities, social service agencies or other providers of concrete servicesCollaborates with interdisciplinary team members to discuss care needs and identify solutions to support clients/consumers/membersAttends planning meetings with service providers to coordinate service plansInitiates written (including clinical and progress notes) and verbal communications, and ensure s that all patient record keeping is in compliance with agency, state, city and federal requirementsParticipates in agency and community programs and education, as requestedParticipates in interdisciplinary team meetings, case conferences, staff meetings, quality assurance meetings, and supervision meetings as requiredQualificationsEffective oral /written/interpersonal communication skills and ability to work collaboratively, on an interdisciplinary teamBachelor's degree in social work, health and human services or a related field preferredTwo or more years of equivalent and relevant experience in a social service agency , OR demonstrated ability to relate to client population OR an equivalent combination of the education and experienceMinimum of one year of experience in a mental health or social service setting with emphasis on family support services and/or family psycho-education requiredYouth Peer Advocate (YPA) and/or Family Peer Advocate (FPA) Certification(s) may be requiredValid New York State driver's license , as determined by operational/regional needs.Bilingual skills may be required as determined by operational needs.Sharing your experience with others who are navigating b ehavioral h ealth and substance use challenges is life changing which is why we welcome you to apply even if you don't meet all criteria Recommended Skills Utilization Management Licensed Clinical Social Worker (Lcsw) Case Management Hospital Information Systems Behavioral Health Social Work Apply to this job. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
